Pre-registration

At Colleton Medical Center, we have a talented registration team available to assist you with pre-registration once your procedure has been scheduled. Please make sure you have your insurance information, email, and emergency contact information when calling to complete your pre-registration.

Pre-registration may be done by calling (833) 257-6028 or online via MyHealth One.

Note: Should you choose to complete the pre-registration process online there will be additional forms that must be signed in registration the morning of your procedure. Pre-registering will greatly decrease the amount of paperwork. If you are registering on a mobile device please scroll down and select the submit button.

Inpatient and outpatient registration

Central Registration is located adjacent to the emergency department at Colleton Medical Center.

To avoid any delay in providing care to the patient, please bring your physician's order, which includes the patient's name, date or range of dates for the procedure to be performed, procedure to be performed, patient's diagnosis and physician's signature.

Schedule a mammogram

Outpatient physical therapy registration

Outpatient physical therapy registration is located in the Physical Therapy Office located inside the Colleton Medical Center Medical Arts Building.

Frequently asked questions

For scheduled elective procedures and tests, if the estimated total due is paid on or before the date of service, a 20 percent prompt pay discount will be applied.
If the patient cannot secure the full amount prior to their scheduled elective services, procedures, or tests, they may pay 50 percent of the estimated due and additional payment arrangements will be discussed. Note: Lower down payments are made on a case-by-case basis.
For those who have recently lost their job and/or are facing a financial hardship, Colleton Medical Center has a financial counselor available to help explore different programs that may assist with the cost of services. These cases must be approved prior to the services being rendered. The Financial Counselor can be reached at (843) 782-2654, during normal business hours.

If you are concerned about the cost of your service before it is scheduled, please contact the pricing office at (888) 334-3868. You will need the following information before you call:

  • Description of the service (i.e., surgery, CT scan)
  • Type of service (i.e., inpatient, outpatient)
  • Your physician or specialist’s name

To use the Patient Payment Estimator, you will need to follow the below steps to retrieve the estimated out of pocket cost:

  • Select the type of service
  • Search for insurance provider
  • Enter your insurance identification number

The final bill covers all hospital services including overnight stays, supplies, nursing care, equipment use, nutritional services and any other services handled by members of the hospital staff that occurred on the facility premises.

However, physicians and other support professionals are independent contractors. Services may be billed separately by the following physicians:

  • Your personal physician
  • Emergency room physicians
  • Radiologists (X-ray and imaging)
  • Anesthesiologists
  • Cardiologists (cardiac testing)
  • Hospitalists
  • Pathologists (laboratory)
  • Pediatricians (newborns)
